Incomplete submissions will not be considered.

Within the local communities surrounding our headquarters locations, our focus for giving is arts and culture, education, healthcare, and social and economic issues.
Outside of our headquarters area, our charitable giving is focused on access to healthcare, and the appropriate treatment of pain, and women's health.as well as supporting programs and services aimed at improving the quality of life for individuals and their families impacted by the following diseases:

If we are requesting support for a fundraising event, how long before the event do we need to request the contribution?
A:
Please submit your contribution request and all required information at least two months prior to when the desired contribution is needed. We will not consider requests under this timeframe.

What is the timeframe for submitting a request?

We accept submissions throughout the year. You will receive notification on the disposition of your request approximately 8-10 weeks from when you submitted your request.

It is our preference that one comprehensive request be submitted reflecting all programs and events which you would like supported in a particular year
